Records Request

Clients and former clients can request their medical and/or billing records in writing sent to the email listed below on this website (caroline@colorfultrailscounseling.com). The records will then be provided to the client/former client, which can be distributed as needed or wanted by the client/former client themselves. We will get back after the written request within 10 business days. We may deny the request if we reasonably believe it would affect your care.
